About Carolyn Horowitz Amacher, MSW, MSG
I founded CHA+ Concierge for Healthy Aging to bring social work values and gerontological expertise to one of life's most complicated transitions: helping people age with dignity while helping families know how to help.
My approach is collaborative rather than directive. The older adult remains a person with preferences, history, relationships, and a voice — not a problem to be managed.
My 30-year nonprofit leadership career has specialized in community engagement and outreach, board governance, program outcomes and evaluation, and professional development and recruitment. I hold a dual BA in newspaper and psychology from Syracuse University, an MSW from Yeshiva University, and a Master's in Gerontology from the University of Missouri, where I also earned a graduate certificate in nonprofit management and the Chancellor's Certificate in Neighborhood Leadership. She recently completed her professional coaching certification with George Mason University's Leadership Coaching for Organizational Wellbeing.

CHA+ Concierge for Healthy Aging is provider-neutral and does not sell home care, senior living, legal, medical or financial products. The role is to help the client clarify goals, understand choices, activate appropriate resources and preserve the older adult's voice.

Our signature entry service. A comprehensive 90-minute assessment across ten aging domains, a prioritized written roadmap, resource and referral list, and a 30-minute follow-up call. Designed to translate complex aging challenges into clear, actionable next steps while preserving the older adult's dignity, autonomy, and voice.
Advisory Hour
Targeted, hourly aging navigation support for older adults and families. Covers research, calls, referrals, coaching, and transition planning — available as needed for focused guidance on specific challenges.
CHA+ Enhanced
Higher-touch monthly support for families managing more complex or active aging situations. Includes more frequent check-ins, care coordination, family meetings, and transition support — designed for clients who need a more hands-on advisory partner.
Facilitated Family Education, Resources and Decision Making
A 60–90 minute professionally facilitated family meeting, an agreed action list and a concise follow-up summary. Ideal for families navigating difficult conversations surrounding driving and at-home/solo living safety.
Family Coaching & Mediation
A facilitated family meeting identifying and coordinating social, educational, health and transportation resources, communications plans, emergency response, and dignified decision-making on aging in place in one’s home.
